1. Request Your Health Care Records
You have the right to request copies of your health care records. To do so:

Please include: Full name, Date of birth, Dates of service, Specific records requested
Your request will be processed in accordance with applicable Texas law and federal privacy regulations (HIPAA).
2. Contact the Licensing Authority
If you need to contact the licensing or disciplinary authority for your provider, please use the following:
